Lactational Amenorrhea Method

A specific, time-limited postpartum method that uses full breastfeeding and the absence of menses to delay the return of fertility.

LAM has three defined criteria. It is not the same thing as breastfeeding in general, and it is intended only for the first six months postpartum.

The Lactational Amenorrhea Method (LAM) is a defined approach to postpartum family planning that combines full or near-full breastfeeding, the absence of menstrual bleeding, and a baby who is under six months old. When all three conditions are met, the risk of pregnancy is low. When any one is no longer met, LAM no longer applies.

Why it matters for fertility awareness

Many people assume that breastfeeding alone prevents pregnancy. The research on LAM clarifies that the protective effect depends on a specific pattern: the baby is being fully or nearly fully nursed, the parent has not had a return of menses, and they are within six months of birth. Outside those conditions, fertility can return at any time, often before the first postpartum period.

How it relates to NFP

LAM is sometimes used as a bridge in the early postpartum period, before transitioning to a fertility awareness based method designed for postpartum cycles, such as the Marquette postpartum protocol or a mucus-based approach with instructor support. It is most useful when its limits are honestly understood.

What it does not mean

  • Breastfeeding alone is not LAM and is not a reliable form of pregnancy avoidance.
  • LAM does not extend past six months postpartum.
  • Pumping, scheduled feeds, long stretches without nursing, or supplementation can change the hormonal picture and break the criteria.
